We perform a comprehensive evaluation that includes behavioral observation, examining the psycho-social history, and reviewing current and past clinical reports from hospitalizations, doctors and other clinical interventions..  Then we assess each person's potential, diagnose any underlying symptoms hindering progress, and develop a treatment plan which gets your family member on the right track for improvement and independence.   

Our treatment plans  are customized to meet the needs of the individual, with clearly defined goals and strategies for both the patient and the family.  We also help coordinate services outside of ReMed that may be recommended for the individual.

Because each brain injury and each person is unique, so is the rehabilitation path. In each case, ReMed meets the patient and family where the need is. 


Outpatient Evaluations

ReMed's Comprehensive Outpatient  Evaluation takes place in one of ReMed's outpatient clinic sites, typically over a two to five day period. It includes:

·        Comprehensive psychiatric screening (including medication and mobility overviews)

·        Neuropsychology

·        Speech Therapy

·        Physical Therapy

·        Occupational Therapy

·        Recreation Therapy

·        Nursing

·        Substance Abuse

·        Case Management

·        Family Consultation

 

Following a Comprehensive Outpatient Assessment, a detailed report is provided to the individual and their family, the referring physician and the insurance representative.  Included in this report is information about the current level of functioning and recommendations for treatment and long term support.

ReMed's Field Assessment takes place in the individual's home and community and addresses issues specific to how the person is functioning in their current living environment and community.  A therapist will follow an individual through their daily activities (including morning routines, household activities, vocational pursuits, school and leisure and recreational activities) and make recommendations for changes to accommodate both the individual and their family for a more successful and productive day to day life.  The Field Assessment can also assist the individual and their family in developing a plan for the individual's future.  At the conclusion of the evaluation, a detailed report which provides information about current level of functioning, future treatment recommendations, strategies that work for the individual and resources for future treatment and long term support is provided to the client and their family, the referring physician and the insurance representative.

Residential Evaluations

For individuals with more intensive needs or for those preparing to live in a more independent environment, ReMed's Comprehensive Residential Evaluation takes place in one of ReMed's residential settings.  This evaluation can range from one week to one month and includes a complete overview of the individual's level of functioning.  The evaluation takes place in an intensive therapeutic milieu and can include evaluations from all therapy disciplines, behavioral analysis, physician evaluations and medication assessment.  This evaluation provides a comprehensive assessment of how an individual is functioning on a 24 hour a day basis - cognitively, physically, behaviorally and socially.  At the conclusion of the evaluation, a detailed report which provides information about current level of functioning, future treatment recommendations, strategies that work for the individual and resources for future treatment and long term support is provided to the client and their family, the referring physician and the insurance representative.
